FLAIR血管高信号征与颈内动脉系统性短暂性脑缺血发作患者短期内发生脑梗死的相关性研究Correlation research between FLAIR vascular hyperintensities and cerebral infarction in short term after transient ischemic attack of the internal carotid artery

摘要(Abstract):
目的探讨颈内动脉系统性短暂性脑缺血发作(transient ischemic attack,TIA)患者出现液体衰减反转恢复序列血管高信号征(fluid-attenuated inversion recovery vascular hyperintensities,FVH)与其短期内继发脑梗死是否具有相关性。材料与方法回顾性分析2010年1月至2019年3月168例在我院首诊为TIA而MRI上未见到梗死灶并持续进行复查的患者MRI图像及临床资料,应用多因素回归分析法分析FVH、颈内动脉粥样硬化(internal carotid atherosclerosis,ICAS)等是否为TIA患者短期内发生脑梗死的危险因素。结果 168例符合条件的患者中,30例患者(17.86%)具有单侧FVH,45例患者(26.79%)发生短期内脑梗死[出现扩散加权成像(diffusion-weighted imaging,DWI)高信号],据此分为R-DWI [+]组和R-DWI [-]组。多因素回归分析显示,FVH (aOR=0.079,95%CI=0.020~0.310,P<0.001)、ICAS (aOR=0.257,95%CI=0.082~0.810,P<0.001)均为R-DWI组的危险相关因素。即使在校正了FVH×ICAS(用以消除FVH和ICAS的交互性)后,FVH仍表现为R-DWI [+]的独立相关因素。结论 FVH是TIA患者短期内发生脑梗死的独立相关因素。
